Preheat your oven to 375°F (190°C).
In a large bowl, combine the ground beef, breadcrumbs, milk, chopped onion, garlic, egg, Worcestershire sauce, Italian seasoning, salt, and pepper. Mix until just combined.
Transfer the meat mixture into a greased 9×13-inch casserole dish, shaping it into a flat, even layer to cover the bottom.
Step 2: Cook the Macaroni and Cheese
In a medium pot, cook the macaroni according to the package instructions, then drain and set aside.
In the same pot, melt the butter over medium heat. Stir in the flour and cook for 1-2 minutes until it forms a smooth paste.
Gradually add the milk, whisking constantly to prevent lumps. Cook and stir until the sauce thickens, about 3-4 minutes.
Reduce the heat to low and stir in the shredded cheddar and mozzarella cheeses until melted and smooth. Season with garlic powder, salt, and pepper to taste.
Add the cooked macaroni to the cheese sauce, mixing to coat the pasta thoroughly.
Step 3: Assemble the Casserole
Spread the macaroni and cheese over the meatloaf in the casserole dish, smoothing it into an even layer.
In a small bowl, mix together the Parmesan cheese, breadcrumbs, and melted butter. Sprinkle this mixture over the top of the macaroni and cheese.
Step 4: Bake the Casserole
Place the casserole in the preheated oven and bake for 25-30 minutes, or until the top is golden and bubbly.
Let the casserole cool for 5 minutes before serving.
