Make the Meatballs: In a mixing bowl, combine the ground beef, finely chopped onion, minced garlic, egg, breadcrumbs, salt, pepper, and grated parmesan (if using). Mix until well combined. Shape the mixture into meatballs, about 1 inch in diameter.
Brown the Meatballs: Heat a large skillet over medium heat. Brown the meatballs in batches, cooking for 5–7 minutes per side until browned. Remove the meatballs from the skillet and set aside.
Prepare the Gravy: In the same skillet, melt the butter over medium heat. Add the sliced mushrooms and sauté until golden brown, about 5 minutes.
Add Garlic and Flour: Add the minced garlic and cook for 1 minute. Stir in the flour and cook for another minute to form a roux.
Make the Gravy: Gradually add the beef broth and Worcestershire sauce, stirring constantly. Bring the gravy to a simmer and cook until thickened, about 5 minutes.
Simmer the Meatballs: Return the browned meatballs to the skillet, stirring to coat them in the gravy. Let them simmer for 10–15 minutes until fully cooked through and tender.
